About Partnership 4 Change

P4C is the forum for effective service user and carer involvement in the joint partnership arrangements for community care. The main focus is to improve joint working and support service user and carer involvement. The forum is made up of chairpersons and representatives from four third sector organisations within North Lanarkshire.

P4C has unanimously agreed that we will have the right person in the right place at the right time, and for the right reason.

P4C will help make the changes that will enhance service provision in our communities within North Lanarkshire both in health and social care.

P4C aims to influence service planning with a view to improving the quality of care within our localities. The changes will not only improve services, but more importantly the lives of those who most need them.

Who is involved?

P4C is the part of the structure where the main focus is to improve joint working and support service user and carer involvement. This group consists of members of four service user and carer organisations:

  • Lanarkshire Links
  • North Lanarkshire Disability Forum
  • North Lanarkshire Carers Together
  • Voice of Experience Forum

These organisations represent older people, people with disabilities, and people with mental health issues and their carers. Both voluntary and statutory services have a duty to influence planning of services where possible to improve the quality of care to the people who most need it.

There is also representation from NHS Lanarkshire and Social Work.

These strategically positioned organisations work jointly to share information, develop practice and influence change. The underlying goal is to achieve positive outcomes for the service users and carers they represent.
